1900-2000: Shift huddle, get my assignment, report, if able look at meds due, labs that need to be drawn, overall plan for patient for the shift. First assessments, introductions, cath care, CBG’s, insulin SS, vent checks, oral care, empty urometer, empty other drains-check outputs. Meds. Check drips, order replacements if needed. Charting. Neuro checks.

2100: Q2h turns, empty urometer, check outputs. Meds. Neuro checks.

2200: Rounds with MD’s. Check outputs, vent checks, restraint charting, neuro checks.

2230: 15 min break

2300: q2h turns, check output, neuro checks, meds.

0000: Second assessments, CBG-insulin SS, oral care, vent check, restraint charting, check output, meds, charting, neuro checks. Charting. Draw labs.

0100: q2h turns, check output, meds, neuro checks. Halfway there.

0130: lunch

0200: check output, vent checks, restraint charting, neuro checks. A good time to do AM CTH if needed, meds.

0300: q2h turns, check outputs, neuro checks. Meds. Baths.

0400: Third assessment, meds, CBG checks-insulin SS. Vent checks, neuro checks, check output, meds, restraint charting, oral care. Charting. Draw labs.

0430: 15 min break, maybe.

0500: q2h turns, check outputs, meds, neuro checks

0600: I&O’s-clear pumps, check weight, vent checks, check outputs, meds, neuro checks, restraint charting, meds.

0700: Day shift arrives, report.

0730: Go home

0800: shower, have a snack, drink tea, play with doggo, go to sleep.

This is a general outline for the day, this does not include phone calls from family, critical lab values, patient call lights, admissions, discharges/transfers, codes, anything that might go wrong. Drip titrations, calling the doctor. Glucostabilizer patients. NIH assessments. Feeding patients. Checking OGT/NGT residuals, hanging new TF bags, priming IV tubing, going to MRI, bedside CXR, calling lab to find out where in the H-E-DOUBLE HOCKEY STICKS OUR BMP RESULTS ARE. ETC, ETC, ETC.
